Printed Application Form
- If you stop by the library, we'll have you fill out a library card application form. You'll need photo ID and proof of address (if your driver's license is correct, it can serve as both).
- If you print it on your own, bring the completed form to the library, along with photo ID & proof of address.

Online Library Application Form
- Complete this online form to apply for a library card.
- We will contact you after receiving the application to finalize the card or solve any problems.
- This is a great option for homebound residents or people with really busy schedules.
Policy
- All library card applicants must be at least 5 years of age.
- Juveniles (under 16) must have a parent or guardian signature on their library card application.
- Juvenile cards must connect to an adult card.

Lost Cards & Replacements
- Lost library cards should be reported immediately.
- Library patrons are responsible for any items checked out on their card unless it has been reported missing.
- Replacement cards can be issued for no charge.
